A four-page, doubled-sided typed contractual document between Dean Martin and Capitol Records, dated December 21, 1951, and signed in black ink on the last page by Dean Martin. Countersigned by Glenn E. Wallichs, president of Capitol Records, and initialed 12 times in ink by Martin.
