A single sheet card dated in February 1962 from the New York State Athletic Commission. The application for a boxing license was filled out in the name of “Cassius Marcellus Clay,” who later changed his name to Muhammad Ali. The card lists all of Clay’s personal information and measurements. The card also features a complete set of the then 20-year-old Clay’s fingerprints and handprints. Singed in the “Applicant’s Signature” box “Cassius Marcellus Clay” in blue ink. The application was made previous to Clay’s 1962 New York State fights against George Logan and Billy Daniels.
